Steps for Obtaining a Leave of Absence - Review the Leave of Absence (LOA) policy.
- Obtain an LOA Form from the Registrar’s Office.
- Schedule an appointment with the following persons to obtain the signatures required by the LOA Form:
- Academic Advisor
- Director of Business Services, or designee
- Bursar, or designee
- Program Director, where applicable.
- Make an appointment to meet with the Dean of Student Affairs.
- When meeting with the Dean, the student will submit the completed LOA Form and discuss the next steps to be taken (including what supporting documentation will need to be submitted).
